The Annapurna scheme has once again been introduced in the ration card by the Gujarat government in which by filling the form you will be able to create NFSA ration card and get all the benefits of NFSA.

Near Mamlatdar Office (Taluka Development Officer’s Office)
The form will also be available from there.
How many documents will be required?
- Attach income sample (Mamlatdar in urban area and Taluka Development Officer in rural area)
. Xerox of ration card
- Aadhaar card of all members
. Xerox of the bank passbook of a member whose name is in the ration card
. Xerox / rental of lightbill
. If there is land in one’s own or father’s name, copy of A-A.
. Example of Revenue Talati cum Minister if he does not have land.
. Xerox of election card.
Who can fill out the form?
Extremely poor and backward class people.
People who used to get grain and now don’t get it or are included in the new government norms.
All Divyang, Ganga Swarupa sisters, beneficiaries of Viddha Pension Assistance Scheme in the state.
Ration card holders of BPL families not covered by NFSA.
Ordinary class rickshaw pullers, daily earning earners.
Construction workers registered with the Construction Workers Welfare Board will also be able to fill out the form.
The Gujarat government announced a few months ago that 10 lakh new families would be included in the National Food Security Act and would be given every NFSA benefit.
The process of filling up the form is likely to continue for the next two months from 1st December 2020, so please fill out the form as soon as possible.
After filling up the form, if you are included in the new Act norm, you will be issued a new NFSA (National Food Security Act) ration card card or an NFSA coin will be affixed on the old ration card. Will come and then you will get regular grain from Gujarat government again.
